WHAT EXACTLY IS A CHRONIC HEALTH CARE PLAN?
An Enhanced Primary Care (EPC) plan, often referred to as a Chronic Disease Management (CDM) plan in Australia, is a program that allows GPs to plan and coordinate the healthcare of patients with chronic or complex medical conditions.
Under the EPC initiative, patients can access 5 Medicare rebates per calendar year, for allied health services. The plan aims to provide a team-based approach to healthcare, involving multiple healthcare professionals, to enhance the overall management and quality of life for individuals with chronic health issues.
